Auto fill data field & assignee of Jira ticket based on description in Zendesk ticket

I set up a trigger in Zendesk to run this workflow every time a Zendesk ticket is created. Could anyone please help me set up above workflow to automatically fill in the data fields and assignee of a Jira ticket based on the description of a Zendesk ticket? Thank you!
